Biomarkers for early detection of post-transplantation rejection.

چکیده
Although transplantation is one of the major medical achievements of the last half century, the shortage of organs and need for long term immunosuppressive therapy limits its usefulness. Advances in stem cell therapy has the potential both to overcome the shortage of organs but also to provide novel ways of reintroducing a tolerogenic state in patients who require life saving transplantation therapy. Understanding mechanisms of rejection which involve both innate and adaptive immunity would allow for novel therapeutic approaches to eliminate or avoid the use of toxic immunosuppressive agents. The evolving era of functional genomics in organ transplantation has been supported by advances in gene profiling, sequencing, proteomics, antibody profiling and bioinformatics. Thus, heralding a new era of intelligent and personalized monitor and therapy. Molecular and cell based biomarkers and now emerging which may be useful to monitor the immune status of the patient and it is anticipated that over the next several years these will detect rejection for immune events before a transplanted organ or cell is damaged. Patterns of genomic biomarkers are also being developed which may predict patients, who achieve tolerance which may not only be useful in the setting of transplantation, but also in patients with autoimmune disease. The ultimate goal of future studies will be to identify markers with sufficient predictive value to improve graft survival, limit graft injury from under immunosuppression and reduce patient morbidity from over immunosuppression. These approaches will also be critical for successful stem cell therapy where it is known that immunologic barriers limit its adoption.

Comparison of Serum Cystatin C and Creati-nine Levels to Evaluate Early Renal Function after Kidney Transplantation
Background: Accurate and rapid assessment of allograft function is essential in renal transplant recipients in order to detect allograft rejection and to monitor drug nephrotoxicity. We aimed to evaluate the usefulness of cystatin C as a marker of kidney allograft function in the early post-transplant period and to compare this value with that of conventional serum creatinine concentration. Met...
متن کاملCombined Detection of Serum IL-10, IL-17, and CXCL10 Predicts Acute Rejection Following Adult Liver Transplantation
Discovery of non-invasive diagnostic and predictive biomarkers for acute rejection in liver transplant patients would help to ensure the preservation of liver function in the graft, eventually contributing to improved graft and patient survival. We evaluated selected cytokines and chemokines in the sera from liver transplant patients as potential biomarkers for acute rejection, and found that t...

Background & Aims: Diagnosis of poor graft function by modern biomarkers is a step to early detection of injuries to the kidneys. Therefore, in this study, serum NGAL was evaluated in renal transplant recipients and compared with creatinine and glomerular filtration rate (GFR) using CKD-Epi equation. Materials & Methods: This case-control study was performed at Imam Khomeini Teaching Hospital,...
متن کاملPrediction of Rejection in Renal Transplantation by Immune Parameters
Background: Monitoring of phenotypic characteristics of T-lymphocytes in peripheral blood is commonly performed to give the clinical parameters in the management of kidney transplant recipients. Objective: To predict rejection in renal transplantation by immune parameters. Methods: 16 non-diabetic kidney transplant candidates (4 females and 12 males, age = 20-65 yr, first time transplant) wer...
متن کاملبررسی سیر تحمل گلوکز و بتادومیکروگلبولین ادرار پس از پیوندکلیه و رابطهی آن با پیشآگهی کوتاه مدت کلیهی پیوندی
Background and Objective: Post-transplant diabetes mellitus (PTDM) is one of the problems occurring after kidney transplantation. This study was designed to detect the probable relationship between the glucose intolerance and serum and urinary concentrations of β2-microglobulin as well as their value to predict acute rejection of the transplant. Materials and Methods: In this cohort study, 40 p...
